During the term of membership on the committee, the Freedom of Information and Protection of Privacy Act requires that the College obtain a signed “Consent to Obtain Information” form from each Program Advisory Committee member prior to using what is considered personal information.

This form shall be completed at the time a new member indicates a willingness to serve on the committee.
