Please feel free to modify the template to suit your needs. However it is important that all of the topics be reflected in the minutes.

Minutes
Please record as much detail as possible regarding plans for following up on recommendations including who is responsible and the associated timeline. A rationale should also be included.

Report to PAC – Dean, Coordinator, Student
This report needs to be filled in and circulated to PAC members in advance of the meeting. This is intended to maximize the time for discussion and provision of recommendations by the external members of PAC.
