Required Co-op Paperwork

Prior to starting your co-op work term, the Co-op Agreement Form needs to be submitted.  Not submitting this information prior to your start date, puts your co-op credit at risk.

  1. Co-op Agreement Form (due prior to you starting your co-op)
  2. Employer Evaluation – Midterm  (due half way through the co-op work term)
  3. Student Evaluation – Midterm  (due half way through the co-op work term)
  4. Employer Evaluation – Final (due two weeks after you have completed 450 hours)
  5. Proof of Hours Worked – Final Pay Stub or Record of Employment (due two weeks after your end date)

Final Co-op Presentation:

  • 5-10 minute presentation in January (Trees, People, Community course) to first year UFT students
