Required Paperwork

Prior to starting your co-op work term, the Co-op Agreement Form needs to be submitted.  Not submitting this information prior to your start date, puts your co-op credit at risk:

1. Co-op Agreement Form (due prior to starting your co-op)
2. Student Evaluation – Midterm (due half way through your work term)
3. Employer Evaluation – Midterm (due half way through your work term)
4. Employer Evaluation – Final (due two after you have completed your work term)
5. Proof of Hours Worked – Final Pay Stub or Record of Employment (due two weeks after your end date)
