Required Paperwork
Prior to starting your co-op work term, the Co-op Agreement Form needs to be submitted. Not submitting this information prior to your start date, puts your co-op credit at risk:
- Co-op Agreement Form (due prior to you starting your work term)
- Student Evaluation – Midterm (due half way through your work term)
- Employer Evaluation – Midterm (due half way through your work term)
- Employer Evaluation – Final (due two weeks after you have completed your 480 hours)
- Proof of Hours Worked – Final Pay Stub or Record of Employment (due two weeks after your end date)
