Optional Co-op Application

Co-op Administrative Fee

 Students participating in the co-op pay a $558.00 co-op fee for administrative costs associated with the co-op placement.

Required Co-op Paperwork:

Prior to starting your co-op work term, the Co-op Agreement Form needs to be submitted.  

  1. Co-op Agreement Form (due prior to you starting your work term)
  2. Student Evaluation – Midterm (due half way through your work term)
  3. Employer Evaluation – Midterm (due half way through your work term)
  4. Employer Evaluation – Final (due two weeks after you have completed your 480 hours)
  5. Proof of Hours Worked – Final Pay Stub or Record of Employment (due two weeks after your end date)
