This form is to be completed if you fall into one of these situations:

• • You have lost your OneCard or it was stolen;
  • Your OneCard is not functional (damaged, etc.);
  • You have already submitted your photo via “Upload your photo” and your One Card is not available for pick up
  • You are having issues uploading your photo via “Upload your photo”
