Required Co-op Paperwork:

Prior to starting your co-op work term, the Co-op Agreement Form needs to be submitted.  

1. Co-op Agreement Form due prior to you starting your co-op work term)
2. Employer Evaluation – Midterm (due half way through the co-op work term)
3. Student Evaluation – Midterm (due half way through the co-op work term)
4. Employer Evaluation – Final (due at the end of the co-op work term)
5. Proof of Hours Worked – Final Pay Stub or Record of Employment (due two weeks after your end date)
