Required Co-op Paperwork

Prior to starting your co-op work term, the Co-op Agreement Form needs to be submitted.  Not submitting this information prior to your start date, puts your co-op credit at risk.

  1. Co-op Agreement Form  (due PRIOR to starting your work term)
  2. Employer Evaluation – Midterm (due half way through your work term)
  3. Student Evaluation – Midterm (due half way through your work term)
  4. Employer Evaluation – Final ((due two weeks after your end date)
  5. Attendance Journal  Proof of Hours Worked – Final Pay Stub, Record of Employment, or Attendance Journal (due two weeks after your end date)

Co-op Blog/Final Report (due in October): 

Upon completion of your co-op you must submit to the Program Coordinator a blog post detailing your summer work and prepare a 5-minute presentation on your work experience.  You can find the details of your Blog/Final Report here:
