Complete a Criminal Record Check or Declaration Form

All Dual Credit faculty are required to provide a clean Criminal Record Check, including a Vulnerable Sector Search, per Ministry of Education, Regulation 521/01 and Regulation 323/03 as a condition of employment.  If you have completed a Criminal Check for the Dual Credit Program in the past 5 years, please complete and return the faculty-offence-declaration-form.

The Criminal Record Check must be presented in person to Kate McGartland.  Do not send original documents or copies.  Should you be delivering a DC course at a secondary school, a copy of the Check may be requested by the school office.

Dual Credit Faculty will be reimbursed for expenses related to the Criminal Records Check. Please submit the original receipt to Jon Taylor for reimbursement of your expense.

Some police agencies require a letter confirming employment from Fleming before they will complete your criminal record check. Please provide your employee contract letter should this be requested of contact Jon Taylor to have a letter provided.

Criminal Records checks must be requested from your local community police or from the Ontario Provincial Police, whichever provides police services for your residential area. Fees vary depending on your home location, and time frame may vary from a couple of days to four weeks.
