Enrolling in Partial Load Group Benefits
Employees who are eligible for Partial Load benefits must complete the Partial Load Group Benefits Enrolment Form.
Instructions:

- Down the Partial Load Group Benefits Enrolment Form
- Complete the form, selecting the benefits you wish to enroll in, including any optional benefits (Vision Care, Hearing Care, Dental Care, Life Insurance)
- Submit the completed form to benefits@flemingcollege.ca within 31 days of your benefits eligibility date
Important Note: Optional benefits must be opted in within 31 days of your eligibility date, or you will not be able to enroll without a break of more than six months between partial load contracts.
Opting Out of Partial Load Group Benefits
Employees who wish to opt out of benefits must complete the Partial Load Group Benefits Enrolment Form, including the Coordination of Benefits section to indicate coverage under another plan.
Instructions:

- Down the Partial Load Group Benefits Enrolment Form
- Complete the form, indicating that you have coverage under another plan, per the Academic Employee Collective Agreement
- Submit the completed form to benefits@flemingcollege.ca
Important Note: If the Partial Load Group Benefits Enrolment Form is not submitted to indicate that you are declining benefits, you will be automatically enrolled in single Extended Health Care. Benefits cannot be declined without submitting the completed form.
